General Information
Please complete the Enterprise Co-op Application form available through the above link. The form may be completed on the screen and then printed, or printed and filled out by hand.
Instructions for completing your application form:
- Once the form is open, click your cursor in the first field to make it active.
- Press the tab key to move through the fields or move through the fields manually with your cursor.
- For drop down menus, position your cursor over the drop down arrow within the box. When the shape changes to a pointer, you can click for a selection to become available and scroll through, clicking to make your choice.
- When you have completed your form, click on the Print button (located in the top right corner of the first page of the form, underneath the Enterprise Co-op logo and information).
Submit the Completed Form:
Once you have filled out the form, send it to the Michelle Berelowitz, the Enterprise Co-op Coordinator using regular mail, in person or scanned through e-mail. Do not send the completed form by email attachment as the form will come through blank.

Experiencing technical difficulties in opening up the application form? Try one of the following:
- Delete cookies and clean your cache
- Try moving the slider bar located at the right to the top
- Download the latest version of Adobe Acrobat (free) and then try opening the forms again
- Make sure you have Adobe Acrobat version 7 or later (our forms use version 9) and that you only have one version of Adobe on your computer, as multiple versions cause problems)
- Make sure that your browser is set to allow pop-ups from all University of Waterloo websites
- Close your browser and try again OR shut down your computer and try again
- The computer you are using may have a firewall to prevent you from opening up our forms; try from a different computer.
